General Description: Works with homeless Veterans, diagnosed with mental illness or dually diagnosed with substance use, referred to the Safe Havens program by Health Care for Homeless Veterans (HCVA), Veterans Affairs Medical Center. Provides an overall support system structure for the clients in the Veterans Affairs (VA) Safe Havens Program and refers them to local agencies and VA centers for vocational, educational, and health and human services.  Supervises VA Safe Havens House Manager and other VA staff such as a Case Manager and provides guidance on the overall operations and management of the VA Safe Havens under the direction of the Program Director.

 

Essential Duties and Responsibilities:

  • Supervises House Manager and Case Manager in the performance of their duties; completes ongoing performance plans as needed and administers annual performance evaluations. Assists House Manager with operational tasks and staff coverage issues as needed.
  • Provides guidance to staff on clinical issues.
  • Assists the Program Director with overall operations and program services provided at the VA Safe Havens; represents the Program Director, when necessary, in their absence.
  • Provides staff trainings, as appropriate.
  • Assesses clients’ needs and appropriate services and assists in implementing service plans. Provides the VA Health Care for Homeless Veterans (HCHV) staff an individualized services plan within five days of admission and discharge summary within two days of discharge. Oversees client satisfaction surveys as required by HCHV.
  • Maintains an active caseload of clients who need intensive case management.
  • Provides a wide variety of client services such as counseling, providing referrals and resources, and providing life skills training to clients such as: managing a budget, diet, health maintenance, legal issues, monitoring medication, and ensuring clients are safe and secure. Applies for entitlement benefits and provides transportation services for clients to scheduled medical and/or social services appointments.
  • Monitors client progress in substance use recovery and mental health status; collaborates with treatment providers to address crises, relapse and harm reduction strategies as needed.
  • Participates in meetings with service providers including physicians/psychiatrists to help resolve conflict and to monitor progress.
  • Coordinates prescribed medications for clients including: arranging refills with pharmacy, picking up refilled and new medications, addressing pharmacy insurance coverage issues, and assisting clients in filling medication trays weekly.
  • Maintains client files and active resource files.
  • Provides documentation to the VA HCHV staff of services provided to each Veteran on a monthly basis, outlining quality and effectiveness of services provided no later than the tenth day of each month.
  • Collaborates with the client and HCHV staff to conduct treatment and discharge planning reflecting a team assessment of health, social and vocational needs, and the involvement of appropriate community resources in resolving problems and setting goals.
  • Conducts psychosocial assessment and maintains all pertinent program paperwork.
  • Completes client data for reporting purposes including accurate and complete data in the Homeless Management Information System (HMIS) – Service Point for any client who is a Montgomery County resident.
  • Complies with VA reporting and data collection system requirements.
  • Coordinates for Veterans to work towards transitional or permanent housing within six (6) months of admission. Submits the required extension paperwork for any stay beyond six (6) months for approval by the VA.
  • Participates in annual homeless events sponsored by the VA Maryland Health Care System and or the DC VA Medical Center, i.e., local Stand Down events.
  • Assesses clients in crisis and resolves conflicts. Develops interventions to resolve problematic behavior of clients.
  • Leads regular staff and house meetings.
  • Participates in weekly peer supervision with Case Managers and Program Director.
  • Acts as representative of MCCH in collaboration with other service providers.
  • Remains available for on-call emergencies and to address incidents in emergency situations.
  • Consults with Program Director on clinical, administrative, and programmatic issues.
  • Other related duties as assigned.

 

 

Required Knowledge, Skills, and Abilities:

  • Master’s Degree in human services related field with clinical licensure. Minimum of five years of experience in social and human services or related field. Supervisory experience.
  • Hands-on experience in social and human services field dealing with diverse populations.
  • Knowledge of principles, methods and procedures of case management.
  • Knowledge of principles, methods and procedures in addiction and dually diagnosed populations.
  • Ability to negotiate and maintain positive relationships with co-workers and clients.
  • Advocate for homeless population.
